we just got this report from bp a couple of hours ago. after four months of investigation with 50 specialists both inside and outside the company, the company says we are not the only ones to blame. that there was no single factors, as they put it, that led to this well blowout and this huge oil spill. they said that it was a sequence of failures and they detailed some of them in this report. several components of the well that failed, the outgoing ceo tony hayward says it was a bad cement job at the bottom of the well. they talk about bad pressure test results that were accepted, both by bp and the owner of the rig, transocean. a slow response team, 40-minute wait after that blowout started to happen, operational failures detailed in this 200-page report. interestingly, bp says it was unlikely that the design of the well contributed to this disaster. that something that was discussed that the company was accused of here on capitol hill during congressional hearings. bp is making 25 recommendations, saying they hope that these recommendat

bp began mobilizing response in accordance with the plan that bp had filed with the former minerals management service. my discussion over the phone with thad allen and with others centered around the potential for the worst-case scenario. the worst-case scenario i describe is that the blowout preventer could have failed and the safety features designed to shut the well could have failed to operate as designed. we began to process the build out to the unified command response. we want to capture and preserve the information as to why this accident took place. the coast guard have a memorandum of understanding dating back to 2009 which has been transferred to the coast guard presently so that both agency investigators dispatched that morning would rendezvous with the survivors sailing ashore. this was to get the inial impressions from the survivors and what might have occurred with this incident as we aggressively stage it brought an array of assets, and elains we wanted toes,, explain our responsibilities. we worked with mms. secretary impala tunnel was briefed and deputy secretary hayes was b

all of this just 200 miles west of that bp explosion last april. and abc's yunji de nies is in louisiana tonight. >> reporter: the fire broke out on the oil and gas platform just after 9:00 this morning, an eerily familiar plume of smoke reaching up into the gulf sky. the 13 crew members plunged into the water to escape the flames, floating together until rescue arrived. one worker was rushed to a hospital. despite occurring just months after the disastrous bp oil well explosion, the platform's owner, mariner energy of houston, was quick to point out the differences. the company says this platform is not used for drilling oil, but to transport it from wells in the water to the shore, using two pipelines. >> no drilling activities under way. this isn't a blowout by any means. >> reporter: bp's deepwater horizon well spewed more than 200 million gallons of oil from a mile down. this platform was operating in shallow water and moves 1,400 barrels of oil a day. it was not affected by the white house moratorium on deepwater drilling. there were initial
