sanjay gupta. sanjay, sorry about the technical difficulties. >> it happens. >> what makes these new guidelines so monumental? >> well, as you mentioned, this is a huge shift, sort of in the way we think about cholesterol and probably heart disease overall. obviously a continued focus on trying to prevent these problems in the first place, but also this idea that look, we have a better idea of how heart disease develops and should more people be on statin medications. it's interesting, a lot of people know their numbers, so to speak. they know what constitutes bad cholesterol. you want your total cholesterol below 200, your hdl, your good cholesterol, above 60, ldl below 100. now, i throw those numbers at you because a lot of people know these numbers but as you pointed out, they're not that relevant anymore in this new sort of paradigm. instead, they're looking at specific risk factors and saying you take a statin if you have diabetes, period. that's it. if you have evidence of heart disease, does

sanjay gupta. what do you make that they're saying about the calculator? >> well, they have been saying it for a while now, the calculator has been in the works for a few years now. basically, what happened in the last several days they greatly expanded a group of people who should be recommended the statin drugs. so if you have diabetes, type 1 or 2, you would be recommended a statin drug, if you have heart disease of any kind, you would be recommended a statin. if your bad cholesterol is above 190, you would be recommended the statin, 10 years of having heart disease, at 7%, that is the one drawing the confusion. what people are focused on. if you put the numbers in on a risk calculator, for a 60-year-old man who has basically normal numbers, cholesterol below 200, for example, he would meet the criteria, he would be recommended the statin drug, even though he has relative normal numbers. i'm 44 now, pretty good numbers, my risk is low, around 1%. but if i do everything right over the next 20 years, my cholesterol stays fine, my blood pressure stays fine

sanjay gupta joins us on the phone from london. you're familiar with the insurance industry as a doctor, as well ace journalist. how well do you think the insurance industry and companies are going to react to this news from president obama that individuals who have been kicked off their plan, because the plans don't meet requirements of obama care, can stay on their plans for an additional year? >> i think it's tough to paint the entire insurance industry with one broad brush. certainly it's a bit disruptive and people already start thinking about whether it's going to have an impact on premiums, cost for insurers, people buying policies next year. but you know, when you talk to these folks -- i've been reporting on this for a long time -- it's a long game here. you know you -- the whole concept of something known as adverse selection, which you've been talking about on your program, this idea that if your adversely selecting, people costing more money, that's not going to work and people have been describing it as this house of c

sanjay gupta is outfront. sanjay, let me start off with, people have already sort of characterized statins a statin aday keeps the doctor away. sort of a tuck everlasting of drugs. but what is the significance of the new recommendation when it comes to the heart? >> first of all this is a tectonic shift as some doctors have put anytime terms of the way doctors will approach heart disease, the number of people likely to be taking the statistic inmedications. i think the big difference is there was some general guidelines in terms of cholesterol numbers. if it was over a certain amount you could be more likely to be prescribed a statin drug. but now they have very specific guidelines. sometimes it's just one risk factor that sort of triggers a potential recommendation. if you're a diabetic, for example, you could be recommended a taint drug. if you have any evidence of heart disease you could be recommended a statin drug. bad cholesterol over there at the bottom because of congenital causes, statin drug. if your
